Discover What's Holding You Back From Playing Your Best

This assessment form is meant to be broad, and to quickly provide you with the big picture in sport psychology. It will help you see numerous peak performance possibilities so your thinking about the mental game is stimulated. This will be the first of many assessments we will perform to pinpoint your strengths, assets, desires and areas for growth so we can formulate a strategic and tactical plan for building a strong and successful mental game. This assessment is very simple and easy to complete, and should take you around 5 minutes.

It will help us begin to narrow down your desires for successful mental game training outcomes, provide a focus for our first coaching session and serve as a launching point for understanding how the mental game coaching process operates.

I believe that mental game training begins the moment you begin completing this assessment. Understanding the mental game, becoming aware of possibilities, knowing yourself, seeing the big picture, and expanding and sharpening your mental game toolkit all begin now.

Before our first coaching session, please fill out this form and press the Submit button at the end of the assessment, to email this back to me. Parents and/or coaches, if you will also be completing this assessment on your child/player, please include your name in the Comments section and do not share the results of your assessment with them until after I have had my first meeting with the player and/or parents.
